Course Policy

All course purchases are final and refunds will not be issued.

However, you are able to reschedule without any additional costs.

No-shows will need to purchase an ADDITIONAL course if the absence is not communicated prior to the scheduled class.



HIPAA Agreement



Students taking our courses may be privy to protected information during their courses. As such, it is the responsibility of the student to uphold the confidentiality and privacy regulations of the Health Insurance Portability and Accountability Act of 1996 (referred to as HIPAA).



The student understands that all patient information, including but not limited to medical condition, any identifying information, and billing and financial data, is to be kept confidential.

The student agrees to use proper disposal methods of any and all patient information, and ensure that it is not accessible or shared with anyone that does not need to know patient information.

The student agrees to comply with ALL privacy policies and procedures, including but not limited to those under the HIPAA privacy rules.

The student understands and agrees that if they violate patient confidentiality by using or disclosing patient information improperly, they may be subject to legal action. The student also understands that if they are found to be in violation of patient confidentiality, they will immediately be removed from the course.

The student understands and agrees that a patient doesn’t necessarily have to be someone in an emergency, and for the purposes of classroom environments, could include but not be limited to staff, coworkers, and any other persons other than themselves.

The student understands that the privacy and confidentiality policies do not end after interaction with OneLife First Aid and CPR.





Harassment and Discrimination





OneLife CPR and First Aid is committed to maintaining a safe and inclusive learning space that supports the social, emotional, and mental health and well-being of all staff and students. The purpose of this policy is to require all staff and students to maintain an educational environment in which harassment and discrimination in any form is not tolerated and to establish procedures for investigating, responding to, and resolving reports of harassment and discrimination.



For this policy, terms are defined as follows:



Discrimination: to treat individuals differently, or to harass or victimize based on one or more known perceived classifications including, race, color, ethnicity, age, religion, sex, sexual orientation, gender identity or expression, ancestry, national origin, marital status, pregnancy, language proficiency, veteran status, disability, and/or other protected classifications A single incident of discrimination may implicate more than one protected class.



Harassment: Any words or behaviors that threaten, intimidate, or demean a person. Harassment is unwanted, uninvited, and unwelcomed, and causes nuisance, alarm, or substantial emotional or mental distress without any legitimate purpose.



OneLife CPR and First Aid will not tolerate any behaviors listed above, or otherwise that causes any student or staff member to harm in any form.



By signing below, you are acknowledging receipt of this policy, and agreeing to conduct yourself appropriately at all times. Failure to do so can result in disciplinary action taken, which may include removal from class.



**Waiver of Liability**



**Event:** CPR Class Participation/ First Aid or other training conducted by Onelife CPR and or its assocaited entities





I acknowledge that I am voluntarily participating in the CPR Class/ First Aid or other training , which may involve physical activities that have inherent risks of injury or adverse outcomes. By signing this waiver, I understand and agree to the following:



1. **Risk Acknowledgment**: I recognize that participation in the CPR Class / First Aid or other training involve risks, including but not limited to physical injury, emotional distress, or other unforeseen outcomes.



2. **Assumption of Risk**: I willingly assume all risks associated with my participation, including any risk of injury or negative outcome, whether those risks are known or unknown.



3. **Release of Liability**: I do hereby release Onelife CPR and or its assocaited entities ,and hold harmless and any other associated parties from any claims, damages, injuries, or losses that I may sustain as a result of participating in the CPR Class/ First Aid or other training .



4. **Medical Clearance**: I confirm that I am in good health and have no known medical conditions that would prevent my safe participation in the class. I understand that it is my responsibility to consult a physician if I have any concerns.



5. **Indemnification**: I agree to indemnify and hold harmless Onelife CPR and or its assocaited entities from any claims arising out of my participation in the CPR Class/ First Aid or other training.
